We are a cloud-based provider of select analytics for chronic health conditions such as Hypertension, Diabetes, Heart Failure and Obesity. Our HIPAA compliant solution provides a comprehensive view of the patient’s overall health status. The Application collects device readings from Patients, evaluates them against Thresholds and alerts approved Providers and Family Members on anomalies detected.

Health care providers can identify and correlate behavioural events, patient adherence and biometric values to completely understand the patient’s needs and risks. It provides an opportunity to proactively engage with the patient, their caregiver(s), family and loved ones to support better health.

How Does it Work?

Patients are given biometric devices such as Glucometers, BP Monitors, Pulse Oximeters, Weight Scales with a communicator that is either a mobile or a small device that plugs into any electrical outlet. They unobtrusively relay biometric and activity-related readings to the platform. Action driven notifications specific to a patient could be configured by each member of patient’s community care such as Physicians, Specialists, Nurses, Care Coordinators, Case Workers, Home Health Providers, Caregivers and Family Members.

HealthLink provides real-time visibility to patient’s state of wellness.
